Lifestyle Medicine Service
A co-designed lifestyle medicine service delivered within North Lewisham PCN, supporting a diverse population and demonstrating measurable improvements across key health and wellbeing outcomes.
Context
The North Lewisham Primary Care Network (NLPCN) serves a large and diverse population, with significant levels of health inequality. The Lifestyle Medicine Service was developed to provide accessible, community-led support focused on prevention, behaviour change and long-term health improvement.
The Challenge
While lifestyle medicine offers a strong evidence base, there was no established model for delivering it in a way that was practical, inclusive and sustainable within a busy NHS setting - particularly for populations at higher risk of health inequalities.
The Approach
Co-designed with the local community to ensure relevance and accessibility
Delivered by Health and Wellbeing Coaches and Nutritionists funded through ARRS
Combination of one-to-one and group coaching sessions
Structured around the six pillars of lifestyle medicine
Open to all adults within the PCN, with targeted outreach to higher-risk groups
Results and Outcomes
133 patients engaged in the service
75% of participants were from populations at risk of health inequalities (CORE20PLUS)
90 patients included in outcomes analysis
Statistically significant improvements across all six self-reported lifestyle measures
Patients were assessed using a combination of biometric measures (including blood pressure, weight and waist circumference) alongside validated wellbeing scores.
