Integrated Neighbourhood Teams
A system-wide rollout of lifestyle medicine services across Primary Care Networks in Lewisham, using an Integrated Neighbourhood Team (INT) model to support localised, population-based delivery.
Context
Integrated Neighbourhood Teams (INTs) bring together multidisciplinary professionals across primary care, community services, mental health, social care and the voluntary sector to deliver more coordinated, proactive care at a local level.
In Lewisham, this model has been used as a foundation to expand lifestyle medicine delivery across multiple Primary Care Networks.
The Challenge
While the North Lewisham Lifestyle Medicine Service demonstrated strong outcomes, scaling this model across a wider system required a structured approach that could be adapted to different populations, teams and local contexts.
