The North Lewisham PCN (NLPCN) Lifestyle Medicine Diabetes Service was developed from the success of the NLPCN Lifestyle Medicine Service which is open to all adults within the PCN. The NLPCN Lifestyle Medicine Diabetes Service is open to all adults within the PCN who are living with Pre-diabetes or Diabetes.
Service Model
The service is run by Health and wellbeing coaches and nutritionists hired via the ARRS PCN funding. The patients have a combination of one-to-one and group coaching sessions working through all 6 pillars of Lifestyle Medicine through a lens of diabetes management.
Evaluation
Patients who went through the service had their: HbA1c, blood pressure, height, weight, BMI and waist circumference measured, alongside self-reported visual analogue scales in each pillar of lifestyle medicine and a validated wellbeing score.
What we found
Patients living with Pre-Diabetes
100% of patients had a reduction in HbA1c.
Average reduction in HbA1c of 5 at 3 months (n=13).
10 patients (77%) had a HbA1c <42.
Average sustained reduction in HbA1c of 5 at 6 months (n=8).
Patients living with Diabetes
100% of patients had a reduction in HbA1c.
Average reduction of 24 at 3 months (n=26).
Average reduction of 32 at 6 months (n=9).
8 out of 31 patients were no longer living with Diabetes (26%).
6 out of the 8 now have an HbA1c less than 42.
