
Integrated Neighbourhood Teams (INT) are multidisciplinary teams working at a neighbourhood level (typically aligned to PCNs) to deliver proactive, personalised, and coordinated care.
They bring together professionals from primary care, social care, mental health, community services, and the voluntary sector to support population health management, reduce fragmentation, and address wider determinants of health — particularly for people with complex or long-term conditions.
INT’s aim to provide the right care, at the right time, in the right place for the right person.

Building on the success of the NLPCN Lifestyle Medicine Service, the Integrated Neighbourhood Team (INT) programme in Lewisham has now commissioned each Primary Care Network to develop and deliver their own tailored Lifestyle Medicine Service.
These PCN-based services are in the process of being set up with Lifestyle Medicine Accelerator and we look forward to sharing more of the results and impact soon.
